How can I not get two surfaces when I convert from Sub-D to polygon?

The main thing to remember is that your Subdivision Surface should NOT be in Poly Proxy mode (make sure there is no low-res cage around your Sub-D object) when converting from Sub-D to poly. Otherwise, you will wind up with 2 surfaces.
