The problem might be that the Pole Vector Control is parented to the Shoulder joint. Since the Shoulder is already connected to an IK Handle, parenting the Pole Vector Control to it may give you undesirable results.

The solution: simply reparent the Pole Vector Control under the corresponding Arm IK Control Curve: “hand (L or R)”.
